As a result of the sudden increase in the nosocomial outbreaks of
MDR-TB, the Centers for Disease Control (CDC) published revised
Guidelines for Preventing the Transmission of Mycobacterium
tuberculosis in Healthcare Facilities in 1994. These guidelines
form the basis of the current policies at YNHH for the prevention
and control of tuberculosis. Fortunately, the MDR-TB epidemic did
not emerge in Connecticut. Additionally, the MDR-TB outbreaks in
such cities as New York, Newark, and San Francisco have abated.
Following the institution of effective administrative controls
(i.e. rapid identification, isolation, and treatment of TB cases),
engineering controls (i.e. negative pressure rooms), and proper
respiratory protection for healthcare workers, the nosocomial
transmission of TB has been halted.
In Connecticut, TB continues to be a public health threat with 121
cases reported to the State in 1999. New Haven County and the city
of New Haven reported 26 and 9 cases respectively. Not unexpectedly
every year at YNHH, cases of active TB are diagnosed and treated.
In 1999, 8 cases of Mycobacterium tuberculosis were identified in
the inpatient setting.
Constant vigilance (THINK TB)
and proper adherence to the infection control policies set forth
below will help to prevent the nosocomial spread of TB and ensure the
proper care of those actively infected.
