Discharge Planning
Notification of Public Health authorities about patients and
personnel with known or suspected tuberculosis is a multidisciplinary responsibility. Tuberculosis is a Category 1 reportable disease which requires both local and state health departments to be notified by telephone within 24 hours of recognition or strong suspicion of disease.
A Reportable Disease Confidential Case Report (form TB-86) must be completed and sent to the local and state health departments. Co-infection with HIV must be indicated on the report form if applicable.
Inpatients The reporting of confirmed or suspected TB among inpatients
is the responsibility of the staff of the Department of
Hospital Epidemiology and Infection Control. Physicians caring for
patients with known or strongly suspected tuberculosis should call
8-4634 to initiate the notification process.
Outpatients
The reporting of confirmed or suspected TB among outpatients in the Emergency Department, the Nathan Smith Clinic, the Winchester
Chest Clinic, the Primary Care Clinic, the Women’s Center, and other
clinics should be the responsibility of the medical director of these areas or the patient’s physician, nurse manager, or his/her designee in the area where the patient is seen.
The Department of Hospital Epidemiology and Infection Control is available for assistance if needed.
Employees
The reporting of confirmed or suspected TB among YNHH employees
is the responsibility of the Personnel Health Service.
