The Commissioner of the Connecticut Department of Public Health (DPH) declares a list of reportable diseases annually. It is the responsibility of the patient's caregiver or designee to report a designated reportable disease upon its recognition or strong suspicion. Reportable Diseases are delineated into 2 classes: Category 1 or Category 2 Reportable Diseases.
Category 1 Reportable Diseases are reportable immediately by telephone on the day of recognition or strong suspicion of disease. On weekdays, reports are made to the DPH and local health department; in the evening and on weekends to the DPH. A Communicable Disease Report (PD-23) or more disease-specific report should be mailed to both the DPH and the local public health department within 12 hours of recognition or strong suspicion of disease. The remaining copy should be filed in the department of origin (for inpatients, Hospital Epidemiology and Infection Control; for outpatients, in the clinic) in a secure location.
Category 2 Reportable Diseases are reportable by mail within 12 hours of recognition or strong suspicion to both the DPH and local public health department. The remaining copy should be filed in the department of origin (for inpatients, Hospital Epidemiology and Infection Control; for outpatients, in the clinic) in a secure location.
