Reporting Requirements
Inpatient Units
Physicians, nurses, and other individuals who are aware of a known or suspected case of a reportable disease should notify the Hospital Epidemiology & Infection Control at 8-4634. After hours, staff can be reached through the page operator at 8-3111.
Be sure to include:
The patient's full name, address, sex, date of birth, race/ethnicity, occupation, unit location, and attending physician.
The name of the diagnosed or suspected reportable disease.
The date of disease onset.
Hospital Epidemiology & Infection Control will investigate the case and file the appropriate Disease Report to DPH and the local public health department.
If applicable to a specific communicable reportable disease, HEIC will compile a list of susceptible hospital personnel, patient roommates, visitors, and others who may have been exposed if appropriate isolation precautions were not in place at the time of diagnosis. After such an investigation, HEIC will take the following actions:
Inform Occupational Health Services of the exposure incident and the departments involved so that potentially exposed workers can be identified and evaluated.
Contact the physicians of any patients who were exposed to a communicable disease for follow-up care.
Notify other occupational health units such as Yale University Employee Health if their workers were potentially exposed so that an appropriate evaluation and follow-up can be provided.
The attending physician of the patient diagnosed with, or suspected of having a communicable disease is responsible for the follow-up and care for susceptible family members and/or visitors who may have exposed.
