Ambulatory Clinics
The completion and filing of all Reportable Diseases for ambulatory care patients newly diagnosed with a Reportable Disease is the responsibility of each individual department. Reporting may be done by the physician, the Patient Services Manager, or designee.
There are several standard forms for reporting.

Tuberculosis Case Report (TB-86)

The YNHH microbiology laboratory does append a PD-23 to microbiology results requiring reporting. However, this does not obviate the caregiver's responsibility if the Reportable Disease is diagnosed via culture results or the Reportable Disease is strongly suspected or diagnosed clinically before the culture results are known.
