Documentation of Contact Precautions

The doctor, nurse or other individual responsible for initiating contact precautions will document the following information in the "History and Progress" section of the patient's medical record:
The date and time that contact precautions were initiated.
The patient's diagnosis and/or clinical signs and symptoms which require Contact Precautions.
The patient's microbiology cultures and other laboratory studies.

The order for Contact Precautions, as all isolation orders beyond Standard Precautions, must be entered into CCSS in addition to recording the information in the "History and Progress Notes" section of the medical record.

To enter CCSS order, after selecting the patient, click on Master Guide.

Then, select "Isolation". To enter the order for Contact Precautions, select "Contact" and then "Enter".



Note: Once the patient no longer requires Contact Precautions, remove the isolation order by selecting "Clear Isolation/Precaution".
