Notification of Intent to Perform a Procedure Involving Anesthesia

Investigators must notify Veterinary Clinical Services (VCS) and Regulatory & Safety
Services (RSS) of their intent to perform procedures involving anesthesia on vertebrate
animals by completing and submitting one of the above forms 48 hours in advance of
initiating the corresponding procedure.
