Housing
The CSN provides an array of residential placement options and comprehensive support services. Programs include shelters, transitional programs, and permanent housing.
Eligibility Criteria:
- CSN services are for clients living in Catchment Area 7 (New Haven, Hamden, Woodbridge, Bethany), who are 18 years of age or older, indigent, and diagnosed with a severe and persistent mental illness that is not primarily the outcome of substance abuse or mental retardation.
- Additional services are available through the SHP for clients who are specifically struggling with homelessness, mental illness and co-occurring substance abuse.
Referrals:
- Referrals for CSN and SHP (Supported Housing Program) housing require that an Inter-Agency Referral Form be submitted by the client's mental health provider. The referring clinician and client will then be invited to attend a meeting with CSN providers to discuss the client's history and treatment needs. A recommendation for housing will be given and the client will be invited to visit the program to learn more.
- For SHP programs, please also fill out the SHP Screening form that is included with the referral form.
- Referrals for Crisis / Respite and Step-down do not require an Inter-Agency Referral Form. Contact the agency directly for a referral.
- Additional residential services, including but not limited to, permanent housing, services for women and children, and programs for persons with HIV, should be made by contacting the following agencies directly: Fellowship Place, Liberty Community Services, and New Haven Home Recovery.

Supportive Housing Program (SHP)
- SHP is for clients who are struggling with homelessness, mental illness and co-occurring substance abuse. This federally funded program provides comprehensive services including transitional specialized housing with a February 26, 2009 supports; and coordinated dual-diagnosis clinical treatment.
- Fellowship Inn (Fellowship Place) - Provides engagement into the system, transportation, breakfast, a morning program, and community living skills training.
- Frank Street (Continuum of Care, Inc.) - Provides specialized transitional housing and support services with 24 hour on-site staffing, utilizing a therapeutic community model with a dual diagnosis focus. Men only.
- Pendleton House (ALSO-Cornerstone, Inc.) - Provides specialized, transitional housing and supportive services with a structured, dual diagnosis treatment focus and outreach/engagement component. Staff on duty 6:00 a.m. - 9:00 p.m, 7 days per week. Coed.
- Sojourner's Place (Columbus House) - Provides transitional housing and support services, with 24-hour on-site staffing. Women only.
Supervised Housing Programs
- Group Homes offer on-site staffing 24 hours per day in a congregate living situation. Designed to serve individuals who have more persistent symptoms and functional limitations.
- New Haven Halfway House (Continuum of Care) Furnished rooms in a spacious house with communal kitchen, dining, living, and recreational space.
- Dwight House (ALSO-Cornerstone, Inc.) Furnished single bedroom in a three floor home with shared living room, bathroom, kitchen, and recreational space.
- Intensively Supported Congregate Apartments offer on-site staffing 24 hours per day and typically serve clients with some independent living skills who may have more difficulty engaging in services.
- Intensive Supportive Living Program (ISLP) (Continuum of Care) Two fully furnished apartments (1 male, 1 female) with private bedrooms and shared kitchen, bathroom, dining room, and living room.
- Park Street Residence (ALSO-Cornerstone, Inc.) Furnished efficiency apartments with sink, refrigerator, and microwave; both shared and private baths.
- Intensively Managed Clustered Apartments offer on-site staffing 12 hours per day, with on call staffing after hours. Typical clients possess basic living skills but require some staff oversight. Apartments are unfurnished and shared with one other person.
Independent Living Supports
- Scattered Site Case Management Provides varying degrees of case management to clients residing in their own apartments. May provide assistance locating appropriate and affordable housing.
- Norton Court (ALSO-Cornerstone, Inc.) - Focus is on symptom management and skill building. Offers staff contact at least 3 days per week.
- Extended Living Program - Team 3 (Continuum of Care) - Focus is on skill maintenance and autonomy. Offers case management at least once per week.
- Shelter Plus Care Provides rental assistance certificates, with an array of support services (e.g., including clinical, social rehabilitation, substance abuse, and vocational rehabilitation) which are made available to tenants by several local agencies.
- Housing Resource Coordination (Continuum of Care) - HRC will assist in locating housing and, when necessary and appropriate, provide and process applications for loan funds that may be used for security deposit and/or process applications for bridge subsidy requests. Contact: (203) 401-2082
Crisis & Respite/ Step-Down
- New Beginnings (Columbus House)- Focus on transitioning people from high-intensity placements, such as hospital stays, to less intensive housing. Contact: (203) 401-4400 (Phone)
- Crisis & RespitFebruary 26, 2009nuum of Care)- These programs offer temporary housing for those people in a crisis, or for those needing a respite from their regular residence in order to manage stress or symptoms.
