Case ManagementCSN case management provides a continuum of recovery-oriented services and links individuals with housing, primary care, entitlements, and comprehensive supports. Services include outreach, violence prevention, community clinical care, engagement, assertive community treatment (ACT) and other programming. Outreach and Engagement Project - Community based program providing outreach to individuals who have experienced barriers to engaging or sustaining involvement in mental health or substance abuse treatments. Case management is often provided in non-traditional settings such as shelters and soup kitchens. Participating agencies include CMHC (lead agency), ALSO-Cornerstone, Columbus House, Marrakech Day Services, and the Hill Health Center. The Assertive Community Treatment (ACT) Team - Program sponsored by CMHC providing intensive outreach, case management and clinical care to individuals who have prolonged forms of mental illness and who demonstrate both severe clinical instability and difficulty adjusting to community living. The Urban Initiative - An intensive outreach and case management program for men and women who are in the grips of chronic substance abuse. The goal of this project is to provide an entry point into substance abuse treatment and community reintegration for some of New Haven's most disenfranchised individuals. Included in this program is the Taking Initiative Center on Whalley Ave. Participating agencies include the Coordinating Council for Children in Crisis, Inc., Columbus House, Eater Seals Goodwill Industries Rehabilitation Center, Inc., and Marrakech Day Services. Coordinating Council for Children in Crisis, Inc - The following free, home based services are offered in order to prevent abuse, neglect and victimization across the life span, strengthen families, and develop collaborative networks to meet the changing needs of individualsFebruary 26, 2009ence Outreach, Neighborhood Victim Advocacy, Family Support Collaborative, Parenting Support and Rights Initiative, New Haven Family Partnership, Group Support, and Community Education. Hill Health Center - The following programs serve African-American women at risk for substance abuse and HIV/AIDS, persons wishing to gain access to public housing, and developmentally disabled children between birth and three years of age: Village of Power/Amistad Village, The Public Housing Primary Care Program, and Early Stimulation/Birth to Three.
