Hill Health CenterHill Health Center is committed to providing accessible, comprehensive and quality primary and behavioral health care to low income and underserved residents in the Greater New Haven area. Hill Health Center is a private, non-profit community health center that was established in December 1968. It provides comprehensive preventive and primary care services from 18 locations in the New Haven area. Services include medical, dental, laboratory, nutrition, nursing, behavioral health, and programs for special populations, such as homeless people and persons with AIDS. ClinicalAdult Psychiatric Clinic (Outpatient 1.1 MH/SA)– Clinic offering adult outpatient, MH/SA, assessment, individual, group counseling, psychiatric evaluation, psychological testing, and medication management. No evening or weekend appointments. Handicap/wheelchair accessible.
Grant Street Partnership (PHP 11.5, IOP 11.2 SA; Outpatient 1.1 MH/SA)– This program offers adult substance abuse partial hospital, intensive outpatient programming and outpatient services. GSP will accept clients with co-occurring disorders. Assessment, individual and group therapy, psychiatric evaluation medication management, case management, bilingual services, and transitional housing for males in treatment are available. Primary care services on site. Evening appointments available. Handicap/wheelchair accessible.
Northside Community Outpatient Services (Outpatient 1.1 MH/SA) - This program offers adult substance abuse and mental health outpatient counseling, assessment, psychiatric evaluation, medication management, and case management. Medical clinic located at same address. Evening appointments available. Handicap/wheelchair accessible.
South Central Rehabilitation Center (Medically Monitored Detox 111.7D) - This program offers a medically monitored inpatient adult detoxification unit, 3 triage beds, and Methadone Maintenance, with access to individual and group therapy. Outpatient buprenorphine treatment available. Bilingual counselors available.
Child and Family Guidance Clinic (Outpatient MH Children) - This program offers child, adolescent, family outpatient mental health and substance abuse counseling, assessment, individual and group therapy, psychiatric evaluation, medication management, bilingual counselors, linkages to pediatric services at same address, psychological/developmental testing . Services are provided at two sites. Evening and weekend appointments available. Handicap/Wheelchair accessible.
Community Health Connection Counseling Services (Outpatient MH/SA 1.1) - This program offers adult mental health and substance abuse outpatient counseling, assessment, individual therapy. Medical clinic at same location. No evening or weekend appointments. Handicap/wheelchair accessible.
West Haven Health Center Counseling Services (Outpatient MH/SA 1.1) - This program offers Adult mental health and substance abuse outpatient counseling, assessment, individual therapy. No evening or weekend appointments. Arrangements can be made for handicap/wheelchair access.
School Based Health Centers - Providing pediatric, social work and health education services to area elementary and middle school children. Hill Health Center SBHCs include: Davis Street, Hill Central, Jackie Robinson, Lincoln-Bassett, Roberto Clemente, and Truman.
Case ManagementVillage of POWER - Providing motivational engagement, case management, a connection to the faith-based community, a Replenishment Center and vocational supports to African American women at risk for substance abuse and HIV/AIDS.
The Public Housing Primary Care Program - Targeting increased access for public housing residents of New Haven, West Haven and Ansonia in need of behavioral health and medical services.
Early Stimulation/Birth to Three - Children birth to three years of age with developmental disabilities have access to home-based, educational, speech-language pathology, occupational and physical therapy interventions. For children who do not qualify for Birth to Three services, outpatient Speech Therapy and Evaluations are available to children ages 0-5.
