At a meeting of the Meriden Board of Health held on Wednesday evening, January 28th, it was voted to ask you to conduct a Health Survey in the City of Meriden.

Will you please send me all the information I will require about this survey and the earliest possible date this survey will be made? I will want to report to the Board at their next meeting.
