    To schedule an appointment in the Yale Neurology Clinics, click on the slip below or send an
e-mail to neurology@yale.edu from your own mail server.  Be sure to indicate that you
would like an appointment in the Yale Neurology Clinics and to include your name and
phone number along with a detailed description of your complaint.

Appointments may also be set up by calling 785-4085 (after hours: 737-5299) or faxing a request to 737-1597.
