MACS (Medical Article Copy Service)

Use this form to request an article from a journal in print format from the Cushing/Whitney Medical Library or from the Kline Science Library. This is a free service to Yale Medical Center faculty, staff, residents, post-docs and students (Medical, Nursing, PA or EPH students).

  • Non-Medical Center Yale affiliates are charged $6 per request.
  • A $6/article fee will also be charged to any Yale patron requesting copies of materials available through Yale’s electronic resources. Prepayment is required. You may use a PTAEO, pay by check, cash or credit card.  If you wish to prepay using a credit card, please fill out this form. If you are unable to fax it back, please bring it to the Administrative Office in the Cushing/Whitney Medical Library, Sterling Hall of medicine (SHM), 333 Cedar Street, room L110.  We are unable to take credit card information over the phone.
