Coagulation Factor Levels

 
Normal Levels
  Factor II 68 % 50-150 %
 
  Factor V 124 % 50-160 %
 
  Factor X 4 % 50-160 %

Interp: factor X deficiency. No evidence for vitamin K deficiency or rodenticide ingestion


Clinical Course - day 1

  • Received 1U PRBC 1h into admission

  • 4 units FFP given immediately after diagnosis of factor X deficiency

  • Started on Premarin

  • Hct rose to 23.5% within 12 hrs

 

Clinical Course - days 2-4

  • One additional unit of PRBCs

  • Received 2 units FFP/day

  • Factor X levels rose to >20%

  • Menstrual bleeding stopped

  • Premarin D/C'd, started on Lo/Ovral

 

Discharge plan

  • Followup in Hematology Clinic

  • Continue Lo/Ovral

  • Resume normal activity but no contact sports

  • Avoid NSAIDs and aspirin

  • The great-aunt with the blood disorder was eventually contacted; she too has factor X deficiency
