  • Prostate carcinoma
    • 5 yrs prior
    • grade 2-3, possible metastases to T11 and L3

  • CLL
    • 2 yrs prior, Stage 0-1, no Rx

  • Iron deficiency anemia

  • Hypertension, hypercholesterolemia

  • Possible Lyme disease
    • treated with doxycycline

  • Macular degeneration

  • Nephrolithiasis excision

  • 5 yrs ago - prostate biopsy
    • no bleeding complications

  • 4.5 yrs ago - Repair of aneurysm, right arm
    • no bleeding complications

  • 4 yrs ago - Hemorrhoidectomy
    • **profuse post-operative bleeding** - required RBC's and platelets (reported at least 20 units total)

  • 3 yrs ago - Inguinal hernia repair
    • received DDAVP pre- and post-procedure
    • no bleeding complications

  • 2 yrs ago - Skin biopsy
    • no bleeding

  • 1.5 yrs ago - Cataract surgery
    • no bleeding

  • 1 yr ago - tooth extraction
    • bleeding required hospitalization
