Deadlines
Yale Residents: January 15, 2008

Non-Yale Residents and Career Physicians:
January 25 , 2008

Eligibility
Physicians-in-training who have completed their internship year of residency training in accredited US programs in Internal Medicine, Primary Care, Family Medicine, Med/Peds, Ob/GYN, or Emergency Medicine by July 2008 are eligible to apply. Those trained in Pediatrics, Preventive Medicine, Pediatrics, and Surgery are eligible to apply for select sites only. For Career Physicians, applicants from US academic medical centers and community-based practices who are board-certified in any specialty are welcome.

Please note: It is recommended that all Yale/ Johnson and Johnson Physician Scholars be covered by malpractice insurance during their International Health Rotation. However, the Yale/ Johnson and Johnson Physician Scholars Program is unable to provide this coverage. All participants are responsible for providing their own malpractice insurance and are encouraged to inquire as to the status of their current coverage when practicing medicine abroad.

Timetable
Applications and supporting letters for participation must be received by January 15, 2008 for Yale residents; and by January 25, 2008 for non-Yale residents and Career Physicians. The fellowship experience will take place between July 1, 2008 and June 30, 2009. All applicants will be notified by mid-February, 2008. The award must be used during this time frame and may not be postponed except under highly unusual circumstances .

Process
Both the Resident and Career Physician applications are available for download in Microsoft Word format to be completed and printed. In addition to the application, both Residents and Career Physicians are asked to submit their CVs as well as two letters of recommendations, one of which must be from a program director if the applicant is a resident or fellow. Yale residents should submit one recommendation which may not be from a Yale/J&J program director. All letters of recommendation must be enclosed with the application and mailed.

Please mail all FOUR components of the application (form, CV, two letters of reference) in ONE packet to:

Ms. Carolyn Karbowski
Program Assistant
Yale School of Medicine
333 Cedar Street
PO Box 208025
New Haven, CT 06520-8025

It is the applicant's responsibility to collect all letters and enclose in application packet. Incomplete application packets will not be reviewed.
To confirm our receipt of your packet, please contact Carolyn Karbowski by telephone, (203) 688-2476, or via email to: Carolyn.Karbowski@yale.edu

Site Selection & Assignment
Applicants choosing an independent site must complete the Independent Site Request Form (available for download below) and include a letter of support from the host institution describing the site as well as work expectations for the applicant. Both the form and the letter must be received or emailed by the application deadline. Applicants requesting an independent site will not be reviewed without this information.

Please note the minimun number of weeks required for each site's rotation. The minimun includes travel time, but the award will be reduced if the minimun time required is not met.

Applications/Online Forms
The following Microsoft Word files include all instructions and can be downloaded to your computer, completed, printed and mailed.

Selection Criteria
Selection is based upon site availability, dates which residents and senior physicians are available for rotations, and desired experiences. Previous international health experience is not a criterion for participation at the resident Career Physician level.
