This 23 year old woman came to medical attention only because of incidental shoulder pain. The chest x-ray at that time demonstrated prominent pulmonary vessels and a proximal pulmonary artery segment. The transthoracic echo demonstrated a secundum atrial septal defect (greater than 1.4 cm in diameter) with obvious left to right shunting on color doppler. Physical exam showed a widely split second heart sound which varied little with respiration. The large size of the defect excluded the possibility of catheter-based clamp closure, thus surgical correction is scheduled.
