Appointments & Promotions Process

If you have questions regarding the appointments and promotions process or about the necessary forms, contact your department or the Faculty Office.

These are the forms that should be submitted to your department regarding appointments and promotions. Please note that your department may require additional materials.

  • Yale-format Curriculum Vitae - template available at Faculty Office website
  • Description of Activities - template available at Faculty Office website
  • Teaching Evaluations - template available at Faculty Office website. In addition, actual evaluations should be compiled by your department.
  • List of referees for letters of evaluation - you and your section chief/lab chief/department chair should discuss these so that the department chair can compile the final list. This final list will not be shared with the candidate.
  • Reprints - select a packet of reprints. Include a description of each reprint and explain why the paper is important.
