Physical Examination

PE reveals a thin woman in moderate distress wanting all lights off in the room.

Vital signs

Temp 101.6 degrees; pulse 112; R 26; BP 140/84.

HEENT

Neck supple; sinuses nontender; oropharynx red but without exudate; conj normal; Fundi reveal sharp discs without retinal exudates; TMs are red without effusion.

Lymph nodes

None palpable.

Chest

Clear.

Heart

RRR with 1/6 SEM at base.

Abdomen

Soft and non-tender; no organomegaly or mass.

G/R

Normal; heme negative.

Extremities

Normal.

Skin

No rash or petechiae.

Neurological exam

Somnolent but oriented; motor, sensory, cerebellar and CN exams are normal; Kernig sign is negative but Brudzinski sign is positive.
