PE reveals a thin elderly man in mild distress.
Vital signs
Temp 98.8 degrees; pulse 110; R 16; BP 108/76.
HEENT
Crusted blood on face and in oropharynx but without visible trauma or mucosal abnormalities; TMs are normal.
Lymph nodes
None palpable.
Chest
Clear.
Heart
RRR with 1/6 systolic ejection murmur at base without radiation.
Abdomen
Soft nontender without hepatosplenomegaly; no ascites is appreciable.
G/R
Normal testes and genitalia; stool dark and heme positive.
Skin
No rash or lesions.
Neurological exam
Alert, oriented, non-focal exam; no asterixis.
