Josh Grimes: Interview
Dr. Grey: Josh, it's so good to see you. What's the matter?
Josh: Well doc, it's like this. I'm light-headed.
Dr. Grey: Tell me more about it.
Josh: I've been working on a new book about the Pox House over by East River. It was built during the Revolutionary War to isolate victims of small pox. The house is gone, but the stone foundation is still there. I've been hiking through East Woods alongside the salt marsh most everyday to look for artifacts and get photos for my book. I've been feeling pretty weak and almost passed out a bunch of times. Same thing happened yesterday at the historical library, just when I stood up. I nearly staggered out of there. People must have thought I was drunk!
Dr. Grey: When did it start?
Josh: Bout a week ago.
Dr. Grey: Any pain in your chest or shortness of breath on those hikes through the woods?
Josh: Nope.
Dr. Grey: Have you noticed any bleeding from the rectum or any black bowel movements?
Josh: No.
Dr. Grey: Any vomiting or diarrhea recently?
Josh: Can't say that I have.
Dr. Grey: I'd like to examine you now Josh.



The correct answer is False. Chest pain is the cardinal symptom of ischemia, and Josh has had none, even when hiking through the woods. How would you classify the possible causes of faintness? What questions would help you to develop a differential diagnosis for Josh?
